New Supervision Agreement Form

Effective January 1, 2022 supervisors and supervisees will need to complete and sign a Supervision Agreement Form. This new form will replace the Supervisor Responsibility Statement and Supervisory Plan forms and will be required for new supervisory relationships from January 1, 2022 forward. If you are currently working under supervisor or begin working under a supervisor prior to that date, the new form will not be required. The BBS has indicated that the new Supervision Agreement Form will be released on January 1, 2022.
